REGISTRATION FOR WORKSHOP:
Please be sure to read all the print below to ensure that you understand all the steps required to register.
Also, be sure to fill out all forms (Registration and Waivers) completely.
THE REGISTRATION FORM:
To register, simply fill out the Online Registration Form below. There, you will be promtepd to read and agree to the Waiver Form and to make your payment. If you do not wish to register online, but would rather print out a copy of the registration form, fill it out and mail it back, you may do so here: 2016 Photo Workshop Registration Form. Download this form, fill it out, and mail it back with your check or money order and filled out Waiver Forms to:

*We must recieve the Registration Form and Waiver Form, both completely and properly filled out, along with your payment, no later than 30 days before the first day of the Workshop in order for your registration to be valid. If these requirements are not met, your spot could be forfeited and you may potentially lose the payment you already sent in if a payment has been received. Please be sure to follow all instructions carefully and to fill all forms out properly. If you have any issues, please contact us as soon as possible. Registrations will not be received after the 30 day deadline.
THE WAIVER RELEASE FORM:
If you are registering online, simply fill out the Online Registration Form below and you will be promtepd to read and agree to the Waiver Form before making your payment. However, even though you agree to it below, you will still need to download the waiver and have it with you, filled out, when you arrive on the first day of the workshop. This only applies if you are registering online. If you are registernig via mail, please download and print a copy of this Waiver Form, fill it out completely, and send it back in the same envelope as your Registration Form and payment.
THE PAYMENT PROCESS:
If you are registering online, simply fill out the Online Registration Form below and you will be promtepd to make your payment via PayPal. If you are registernig via mail, please send your payment in the same envelope as your Registration and Waiver Forms. Your payment can either be by check or money order. No cash please.

Do you have any known allergies (bug bites, foods, medications, etc), or health conditions that we should be alert to? Please describe.
Only check one of the options below if it applies to you. You can only check one. If you are Active Duty Military Personnel or a First Responder (Firefighters, Police, etc), you must present some form of proof upon arrival the first day of the workshop. If you are an accompanied minor (9 - 17 years of age), you must present proof of your age and prove that you are accompanied by an adult 18 years or older. Violators will be sent home and will lose their payment.
